999 Seafood Supermarket, 5315 Gibson SE (Class 4, Class C)

Minimal rodent droppings remained present within the grocery aisles. A rodent dropping was observed directly on the bulk rice bags. Rodent droppings were present in the back stock storage area for dry goods. Droppings accumulated on boxes of chopsticks and trash bags. Significant debris accumulation was noted throughout the aisle shelving.

Amadeo’s Pizza & Subs, 809 98th SW

The can opener, bulk storage tubs and deli slicer were soiled with dried food residue. A five-gallon working bucket of bleach sanitizer water lacked a label identifying its contents. The facility had an active cockroach infestation that failed to be controlled. The person in charge failed to demonstrate basic food safety knowledge regarding the six reportable illnesses. The three-compartment sink faucet was missing its handle and could not be turned on and off properly. The grease trap leaked grease and liquid waste sludge onto the kitchen floor. Kitchen ventilation intake and exhaust air ducts had an accumulation of debris. Raw shell eggs were stored directly above ready-to-eat romaine lettuce inside the walk-in cooler. Ambient thermometers were missing or inaccessible in multiple cold-holding units. The facility was found operating with an expired permit. Clean single-use cups, lids, and containers were stored directly on the floor. Cockroaches were using the floor-stored single-use containers as a harborage area. A vegetable oil container was improperly reused to store a bulk powdered ingredient. Floor and wall juncture coving was absorbent, damaged and difficult to clean. The building had missing floor tiles, damaged wall attachments and multiple unsealed holes throughout. Required temperature logs were unavailable for the stored food products.

Einstein Bros. Bagels, 8060 Academy NE

Substantial debris built up inside the back ice machine. Plastic cutting boards provided unsafe and non-smooth food contact surfaces. The three-compartment sink faucet could not shut off completely. The soda machine leaked a substantial amount of water onto the floor. Kitchen floor drains had a buildup of debris. Food employees lacked required food handler cards. Kitchen ventilation intake and exhaust ducts had a buildup of debris. A working container of flour or corn meal lacked a common name label. Synthetic wood shelving inside storage cabinets was damaged and unsealed. The men’s restroom was out of order because the toilet failed to flush. No chemical test kit was available to measure sanitizer concentrations. An employee wore unapproved bracelets while preparing breakfast sandwiches. The facility operated with an expired permit and an unverified business registration. No food permit was conspicuously posted on the premises. The three-compartment sink lacked cold water and only provided hot water at 140 degrees. Quat sanitizer was improperly mixed with only hot water due to the missing cold water. Employees completely failed to wash their hands during the entire inspection. Cashiers prepared drinks and line cooks donned gloves without washing their hands. Line cooks cracked raw eggs and then handled sandwiches without washing hands. Debris built up inside cabinets, under equipment and on kitchen shelving.

Hilton Garden Inn, 5320 San Antonio NE (Class C)

An extreme buildup of black organic matter was observed inside the kitchen ice machine. Chopped green chile, sliced tomatoes and cut squash exceeded the seven-day storage limit. Several floor drains located throughout the kitchen lacked an adequate plumbing air gap. Sliced tomatoes, chopped green chile and Greek yogurt were held at unsafe cold-holding temperatures. The upright cooler near the cook line was insufficient and failed to maintain proper temperatures. The facility lacked a chemical testing kit or device to measure sanitizer concentrations. The handwashing sink located in the bar area lacked a required handwashing reminder sign. A back-of-house handwashing station was blocked and rendered inaccessible by a mop bucket. An employee wore unapproved items of jewelry while actively preparing consumer food. A soiled mop was stored improperly by being left directly on the floor while not in use.

Hot Dog Man, 314 Iron SW

Chlorine sanitizer in a spray bottle and wiping cloth container measured 0 ppm. The mobile unit lacked a testing kit to measure chemical sanitizer concentrations. No hand soap was available at the handwashing station. The handwashing sink lacked an approved hand-drying method. The handwashing sink lacked a required handwashing reminder sign. The handwashing sink was used improperly to store hand tools and discard drinks. An employee did not wash hands when switching tasks and donning a new glove. An employee used a soiled cloth and dirty water to wash hands without cleanser. A plastic working spray bottle containing an unknown chemical lacked a label. A food handler was observed working with unkempt fingernails. An employee prepared food inside the unit without a proper hair restraint.

Kobe Pho, 10200 Corrales NW

The deli slicer in the rear kitchen was soiled with food debris. Various broths, chopped vegetables, prepared meats and house-made sauces lacked required date markings. The person in charge was unable to provide required employee food handler cards. Ventilation intake and exhaust air ducts above the prep area had a buildup of debris. Chopped fruit, broth and sliced onions were held at unsafe temperatures inside the reach-in cooler. Ambient thermometers were missing or inaccessible in multiple cold-holding units. The facility failed to provide a designated area for employee dining. No lockers or suitable storage facilities were provided for employee clothing and personal items. Front and rear kitchen handwashing sinks were blocked by kitchen equipment and soiled utensils sitting inside the basins. Dirt and debris built up on kitchen floors and walls, especially underneath equipment. Clean single-use deli cups were stored improperly directly on the floor at the server station. Fryers, the grill top, reach-in coolers and storage shelving were soiled with grease and food debris. Required temperature logs were unavailable for hot and cold temperature-controlled foods.

SpringHill Suites, 5910 Holly NE (Class B)

The quaternary ammonium sanitizer in the food prep area measured 0 ppm. The required chemical sanitizer solution was not prepped for the day. Sliced ham and sliced cheese inside the upright cooler lacked required date markings. The person in charge failed to demonstrate necessary food safety knowledge. The floor drain near the warewash machine was in disrepair and missing a strainer. The floor drain near the warewash machine lacked an adequate backflow air gap. Food portioning containers for nuts and coconut flakes in the self-serve area lacked common name labels. A food employee prepared food for the self-serve area without a proper hair restraint. An employee failed to wash their hands when switching tasks and donning new gloves. The facility lacked required calibration logs for its temperature-measuring devices.

The Paleta Bar, 12501 Candelaria NE

Dried food and soil debris accumulated on a can opener blade in the back prep area. A working spray bottle containing an unknown chemical lacked an identifying label. Working containers holding spices and an unknown white food product completely lacked required common name labels. Butter, cotija cheese, diced honeydew and diced cantaloupe were held at unsafe cold-holding temperatures. Required food temperature-measuring devices were not provided or readily accessible. The facility lacked a functional sanitizer testing kit, and its available test strips were expired. The back handwashing sink in the kitchen prep area lacked an approved method for hand drying. One front handwashing sink was blocked by a towel left inside the basin. Another front handwashing sink was completely blocked by a broom, dustpan and squeegee. Chemical sanitizer in a chlorine wiping cloth bucket exceeded the maximum limit at 200 ppm. An employee wore an unapproved watch while actively handling open food items. A food employee prepared open food items in the kitchen without a proper hair restraint. A soiled mop was stored improperly by being left directly on the floor while not in use. Buildup and debris accumulated on the floors and walls across the kitchen prep area. The facility failed to provide written allergen warnings for its unpackaged food items. Food scoops and cups were stored with handles directly touching the food products. Food and soil debris built up inside a front floor drain and on the three-compartment sink sprayer. The sides of the ice cream machines had a noticeable accumulation of soil residue. Organic growth was found inside a non-working cooler unit located behind the front counter. The wall next to the front handwashing sink was in disrepair with a golf ball-sized hole. The establishment completely lacked required temperature logs for its stored food products. No calibration logs were maintained for the facility’s temperature-measuring devices.
