Empire Board Game Library, 3503 Central NE

Black organic matter built up on the interior of the back-of-house ice machine. The floor drain under the three-compartment sink lacked an adequate backflow air gap. The establishment did not have a certified food protection manager on staff. Food employees completely lacked required food handler cards. The toilet room lacked a tight-fitting, self-closing door or was left propped open. Sliced turkey, pre-cooked chicken and pre-cooked bacon were held at unsafe cold temperatures. The facility lacked a current testing kit to measure chemical sanitizer concentrations. No food thermometer was available to check accuracy at the sandwich-making station. An employee wore an unapproved wristwatch while actively handling food. The facility was found operating with an expired permit. A soiled mop was stored improperly directly inside a mop bucket instead of being hung to dry. Required temperature logs were unavailable for stored food products. The facility completely lacked calibration logs for its temperature-measuring devices.

Freddy’s, 10201 Central NE

Quaternary ammonium sanitizer solution on the main cook line and behind the front counter measured 0 ppm. The interior of the ice machine bin had a substantial organic buildup. The can opener blade was soiled with a buildup of food and soil debris. Floor drains were in disrepair due to missing protective mesh or dome covers. Floor drains at the drive-thru and the back door lacked adequate backflow air gaps. A working container holding seasoning lacked a required common name label. Sautéed onions, sautéed mushrooms, relish, sliced tomatoes, diced onions, diced tomatoes and shredded lettuce were held at unsafe cold temperatures. The facility failed to provide a designated area for employee dining or tobacco use. Employee water bottles were stored inside the walk-in cooler directly above facility food items. Handwashing cleanser was unavailable at the hand sinks by the manager’s office and behind the front counter. The handwashing sink by the warewashing area lacked an approved hand-drying method. A soiled mop was stored improperly by being left directly on the floor while not in use. Buildup and debris accumulated on the kitchen floors and walls under and behind equipment, tables and shelving. Food service pans were stacked wet instead of being allowed to air dry. Nonfood contact surfaces on fryers, coolers, the warewashing spray nozzle, TV screens, walk-in fan shrouds and soda machine cabinets had a buildup of food and soil debris.

Homewood Suites, 7101 Arvada NE (Class C)

Brown organic matter built up near the ice chute of the kitchen ice machine. An inadequate plumbing air gap was observed near the warewash machine. The establishment did not have a certified food protection manager on staff. Food employees completely lacked required food handler cards. A metal can of sliced jalapeños was dented and compromised. The handwashing sink on the kitchen south wall lacked an approved hand-drying method. A mop bucket blocked access to the kitchen south wall handwashing sink. Required temperature logs were unavailable for hot and cold stored foods. The facility lacked calibration logs for its temperature-measuring devices.

Icon Cinema, 4591 San Mateo NE

The ice cream well sink drain pipe lacked an adequate backflow air gap. The establishment did not have a certified food protection manager on staff. Unsealed wood shelf organizers at the front register and popcorn areas provided porous, absorbent surfaces. Hot dogs and containers of drained sliced jalapeños were held at unsafe cold temperatures. The upright cooler in the warewashing area failed to maintain required cold-holding temperatures. Containers of diced green chile developed an unsafe white film on top. Cleaned wood shelf organizers were stored improperly, less than six inches above the floor.
