Bamboo, 7202 Menaul NE

Live cockroaches were present in the warewashing area. Live cockroaches were found in the dining room service area. Numerous dead cockroaches had accumulated inside the facility. Walls, shelving and the sides of equipment had a buildup of debris. The interior of slicing equipment still had debris accumulation. Holes, gaps, cracks and crevices were noted throughout the facility.

Eat My Thai Cuisine, 5303 Gibson SE

Plates, bowls, knives, a produce slicer and the ice machine deflector in clean storage were soiled with food residue. More than 20 dead cockroaches had accumulated throughout the facility. Ready-to-eat TCS foods in all cold-holding units lacked required date markings. A food employee touched chopped vegetables and prepared meats on the cook line with bare hands. The person in charge failed to demonstrate adequate food-safety knowledge. Multiple plumbing faucets and drain pipes were leaking and in disrepair. Floor drains under the three-compartment sink and dishwashing machine lacked proper air gaps. The establishment lacked a certified food protection manager on staff. Interior ventilation intake and exhaust air ducts had substantial debris buildup. Batter, sliced vegetables, soups and pot stickers were stored uncovered in coolers. Broken utensils, unused dishes and nonfunctional kitchen equipment accumulated on the premises. Chopped onions, pot stickers, green onions, sliced carrots, chicken chunks and sliced cucumbers were held at unsafe temperatures. Bulk cases of rice, curry paste and sauces were stored directly on the floor. Food temperature-measuring devices were not provided or accessible. No hand soap was available at the boiler room sink, and a heavily watered-down soap mixture was used at the make line sink. Both kitchen handwashing sinks were blocked and inaccessible due to stored supplies and equipment. A food employee peeled shrimp in the kitchen without wearing proper hair restraints. Kitchen floors, walls and the cook line had a heavy accumulation of grease, debris and food residues. The facility failed to maintain any required temperature logs for stored food products.

KFC, 12921 Central NE

All quaternary ammonium sanitizer buckets were empty despite the store being open for business. Soda machine nozzles in the dining room had a buildup of syrup and debris. Clean food service pans in the kitchen were soiled with dried food and soil debris. Two to four dozen dead cockroaches accumulated in the kitchen and dining areas. Coleslaw and pot pies in the walk-in cooler lacked required date markings. Floor drains under the front counter and near the drive-thru window lacked proper air gaps. The eastern outer front doors had small holes at the bottom that let light inside. No sanitizer testing kit was available in the facility. The facility did not provide a designated area for employee dining or tobacco use. A soiled mop was stored improperly directly on the floor. Substantial debris built up on kitchen and dining room floors and walls under shelving and equipment. Food service pans were stacked wet in the warewashing area instead of being air dried. The back plates of the soda machines had heavy syrup and debris buildup. The sides and backs of the fryers had substantial grease buildup. Dining room tables and chairs had an accumulation of dried food and soil debris. The sides of the three-compartment sink had heavy debris buildup. Wax paper holders on the main line had heavy debris buildup. Gas, water and electric pipes in the kitchen had substantial grease buildup.

Kokio Korean Fried Chicken, 8019 Menaul NE

Working spray bottles containing sanitizer and coconut oil lacked required identifying labels. Prepared potatoes, kimchi, sliced cabbage and sliced onions lacked required date markings. The facility did not have a certified food protection manager on staff. Food employees did not have valid food handler cards. Kitchen containers holding flour and bulk spices lacked required common name labels. Cardboard used to cover kitchen shelving was torn, damaged, absorbent and not smooth. A stock pot of teriyaki sauce was stored improperly directly on the kitchen floor. The establishment failed to provide a designated area for employee dining. Handwashing cleanser was unavailable at the kitchen handwashing sink. The kitchen handwashing sink was blocked by a large pot of teriyaki sauce. Required temperature-measuring devices were missing from the cold-holding units, reach-in floor freezers and standing freezer. The facility was found operating with an expired permit. The food establishment permit was not conspicuously posted inside the facility. Required temperature logs were unavailable for stored and received TCS food items. The establishment completely lacked calibration logs for its temperature-measuring devices.

Which Wich, 8110 Louisiana NE

Severe debris and grease built up on a conveyor-style toaster oven. Dried food debris accumulated on a prep table can opener blade. A restroom toilet was in disrepair and unable to flush. A three-compartment sink sprayer nozzle leaked, causing standing water to seep into the dining area walkway. The establishment did not have a certified food protection manager on staff. Food employees completely lacked required food handler cards. Metal wire shelves inside the walk-in cooler were rusty and dirty. Sliced tomatoes, cucumbers, shredded lettuce and red onions were held at unsafe cold temperatures. The prep top cooler had insufficient holding capacity, requiring ice containers to be placed inside. The walk-in cooler had inadequate lighting, requiring the inspector to use a flashlight. The handwashing sink in the south restroom had inadequate water pressure, running only at a trickle. Dried food debris and ice built up on the walls and bottom of the reach-in freezer. Required temperature logs were unavailable for stored, thawed and cooked food items. The facility lacked calibration logs for its temperature-measuring devices.
