Fiesta Park Wellness & Rehabilitation, 8820 Horizon NE
Food debris and buildup were found on the can opener blade. Pest control procedures for rodents were inadequate. Employees improperly cleaned up rodent droppings. Substantial rodent droppings were found throughout the facility, including on plates, bowls, prep shelves, cold-holding equipment, storage areas and kitchen floors. Plumbing piping was leaking and not maintained in good repair. Food employees lacked valid or unexpired food handler certifications. A handwashing sink was blocked by a tray of cups stored in the basin. The establishment operated with an expired permit. Food debris and buildup accumulated on floors under tables, shelves, cooking appliances and dishwashing equipment. Spilled or melted red liquid was left inside the undercounter cooler. Floor-to-wall juncture coving tile was missing or removed from the wall.

Golden Pride, 1830 Lomas NE
The chlorine sanitizing solution in the warewashing machine failed to meet required concentration parameters. No sanitizer wiping buckets were available to sanitize food-contact surfaces throughout the facility. Significant debris accumulation was observed on the interior ice chute of the ice machine. Heavy debris accumulation was present on the warewashing machine. Minimal live cockroaches and flies were observed in the kitchen due to inadequate pest control procedures. Green chile stew and beans were held below the required minimum temperature for hot holding. Prepared ready-to-eat foods in the walk-in cooler lacked required date-marking indicators. Open salad dressings in the walk-in cooler exceeded the seven-day storage limit. Walk-in cooler door gaskets were in poor repair. The person in charge failed to demonstrate required knowledge of foodborne disease prevention. An inadequate air gap was provided at the floor drain in the back kitchen area. Severe debris buildup was observed on air vents and ventilation ducts throughout the kitchen. Containers of dredging brine, flour and shredded lettuce were stored uncovered. Exterior doors, screen doors and gates were not tight-fitting and allowed light through the edges. Structural gaps and holes were present along the exterior back door area, drive-through, and west end of the building. Chicken breading brine was stored directly on the floor. Handwashing sinks were improperly used as dump sinks and to rinse a thermometer. An employee wore unapproved wrist jewelry and bracelets while handling open food. A food employee handled open food without wearing proper hair restraints. A food employee failed to wash hands or change gloves after touching dirty ice buckets and their face. Heavy buildup and debris were present across the floors, walls and ceilings. Written notifications of major food allergens were missing for unpackaged food items. Ice scoops were stored directly on a soiled grate. Food pans in the prep top cooler were stored directly under leaking water lines and dirty air vent condensation lines. Nonfood-contact equipment surfaces throughout the facility had heavy debris buildup. Walls and ceilings had gaps, cracks, holes and peeling materials. Kitchen floors and ceilings had damaged aggregate flooring, broken tiles, and missing tiles that were not easily cleanable.

Helen鈥檚 Bakery, 1014 San Pedro NE
Live and dead cockroaches were found in several locations throughout the facility with no control methods in place. Over 30 dead cockroaches and over 20 living cockroaches accumulated in food prep areas, cooking equipment and inside cold-holding units. Red and green salsas in the upright cooler lacked required date-marking indicators. The floor drain under the three-compartment sink had standing water and drained slowly. A water faucet valve in the back room was leaking. The establishment lacked a certified food protection manager on staff. The hood ventilation system allowed grease and condensation to accumulate and drip onto cooking equipment. Red salsa, green salsa and milk in the upright cooler were held above required cold-holding temperatures. The facility operated with an expired permit. A food employee failed to wash hands when switching tasks and donning new gloves. Floor-to-wall juncture coverings had missing coving and damaged sheetrock in several locations. The establishment did not maintain temperature logs for temperature-controlled food items. The establishment did not maintain calibration logs for temperature measuring devices.

Olympia Cafe, 2210 Central SE
Multiple kitchen tools and equipment, including the can opener, ice machine, slicer, shaver and utensils, were heavily soiled with food debris. Working containers of chemical sanitizer were not labeled with their common name. Over 30 dead cockroaches accumulated throughout the kitchen, storage rooms, and boiler room. Rodent droppings were found in the boiler room and dry storage area. An open container of ricotta cheese lacked required date-marking indicators. The person in charge failed to demonstrate basic knowledge of food safety. The walk-in cooler refrigeration condenser was leaking water. An outdoor drain cleanout was left uncovered and filled with trash. The establishment lacked a certified food protection manager on staff. Food employees did not have valid food handler cards. Bulk food containers in the kitchen and dry storage area were not labeled with common names. The rear exterior door was not tight-fitting and allowed light through the edges. Structural gaps and holes ranging between 1-4 inches were present along exterior building walls. Broken equipment, old display cases and unused cooking items accumulated on the premises. Raw meat in cardboard packaging was stored in direct contact with undrained ice. The facility operated with an expired permit. Food employees failed to wash their hands when switching between cleaning, servicing floor drains, dishwashing and handling food. A fresh-water hose was submerged directly inside stagnant water in a backed-up floor drain. A backed-up floor drain under the dishwashing machine caused dirty stagnant water to accumulate. Scoops were stored completely submerged inside bulk food and ice containers. Holes, openings and water damage were present across kitchen walls, baseboards and ceilings. Kitchen floor tiles were missing or damaged throughout the facility. The establishment lacked temperature logs for hot- and cold-held food items.
