Kitchen knives, knife magnet and ice maker had a buildup of food debris and organic matter. Self-serve buffet items 鈥 creamer, milk, green chile and yogurt 鈥 lacked required time-holding labels. The handwashing sink in the dish pit was leaking severely. An inadequate air gap was present at the floor drain next to the ice maker. Exterior doors were not tight-fitting and allowed light through the edges. Lobby self-serve cold items 鈥 strawberries, melon, sliced cheese, and yogurt 鈥 were stored above 41 degrees. The juice machine, walk-in cooler vent covers, fryers, shelving and door handles had soil residue buildup.

Kap鈥檚, 5801 Central NE
Dead cockroaches were found on the floor near cooking equipment. Pest control treatment was not properly maintained. Nonfood contact surfaces were not cleaned at the required frequency.

Love and Care Child Development Center, 5407 4th NW
No chlorine sanitizer solution was prepared at the time of inspection. Dead cockroaches and mouse droppings were found throughout kitchen cabinets, sinks, stoves and refrigerators. Pest traps were improperly maintained, including sticky traps full of roaches and an unset spring trap. Cooked macaroni and cheese, pinto beans and steamed broccoli lacked date markings. Macaroni and cheese was stored uncovered in the refrigerator. The facility lacked a certified food protection manager on staff. The ventilation hood had heavy grease buildup, allowing grease and condensation to drip onto food. Raw shell eggs were stored next to ready-to-eat foods and juices. A wall opening behind the refrigerator left the facility exposed to outer gaps. No approved hand-drying method was available in the employee restroom. The kitchen handwashing sink was rendered inaccessible by buckets stored inside it. Floors under the stove, freezer and refrigerator had heavy buildup and debris. Nonfood contact surfaces, including stove bottoms and lower cabinets, were heavily soiled with debris, dead roaches and rodent droppings.

Quesadilla Way, 5305 Gibson SE
The establishment has been allowed to reopen under unsatisfactory following submission of photographic evidence and in-person walkthrough.

Saint Martin鈥檚 Center, 1201 3rd NW
Simple Green cleaner and degreaser was stored directly above utensils. Cooked pasta in red sauce was stored in the upright cooler without required date markings. The facility did not have a certified food protection manager on staff. Dented metal cans of sauerkraut, baked beans and chicken noodle soup were found on the food storage rack. No chlorine sanitizer was available in the facility at the time of inspection. No chlorine testing strips or devices were available to test sanitizer concentration. No thermometer was provided in the upright cooler to monitor temperatures. No temperature logs were maintained for cooked beans stored in the upright cooler. No calibration logs were maintained for temperature-measuring equipment.

Weck鈥檚, 5115 Central NE
Orange organic matter buildup was observed on the ice machine deflector plate and chute. Aspirin was improperly stored above a prep top cooler. Sliced ham, sliced turkey and sliced cheese in the undercounter cooler lacked date markings. A cell phone was stored on a cutting surface where toast was being prepared. Floor drains under the beverage dispenser and ice machine had inadequate air gaps. Sweet potato fries near the drop-in freezer were left uncovered. A hood vent light bulb above the cooking equipment was burnt out. No ambient air thermometer was present in the upright cooler. An employee wore a wristwatch while working on the cook line. An employee failed to wash hands when switching tasks and putting on new gloves. A soiled mop was left sitting directly on the floor and inside a mop bucket.
