The chlorine sanitizer concentration in the three-compartment sink and wiping cloth container was measured at zero PPM. Food debris buildup was found on the cutting blade of the can opener in the back-of-house prep area. Frozen chopped green chile was found improperly thawing in a bucket of standing hot water. An inadequate air gap was observed at the floor drain located under the three-compartment sink. A dented metal can of sliced jalapeño peppers was found on a storage rack in the back of the house. Cooked beans were stored uncovered in the walk-in cooler, exposing them to potential contamination. A food employee failed to wash their hands before donning new gloves and switching tasks. A plastic food scoop was stored in a dry storage bin with its handle in direct contact with the food. The establishment lacked required temperature logs for stored cooked ground beef. The facility did not have calibration logs available for its temperature-measuring devices.

Brown and yellow buildup was found in the ice machine, and rodent droppings were observed in a stock pot. Sealant and expanding foam were stored on a rack directly next to and above clean food service pans and utensils. Working spray bottles containing degreaser and detergent were not labeled with the common name of the material. No date marking indicators were found on any ready-to-eat food items held within the prep top coolers. The warewashing sink was found in disrepair, leaking gray water directly onto the floor. An inadequate air gap was observed at the floor drain located in the bar area under the three-compartment sink. Food employees did not have valid food handler cards. Plastic containers holding spices were not identified with the common name of the food. An unsealed, absorbent wood panel was installed between the prep sink and the wall. Raw chicken was stored directly above ready-to-eat sauce in the walk-in cooler near the bar area. The toilet room used by females lacked a covered receptacle. Food temperature-measuring devices were not provided or readily accessible for use. The facility did not provide a designated area for employees to eat, drink or use tobacco products. No lockers or suitable storage facilities were provided for employee clothing or personal belongings. Soil residue buildup was observed on the handwashing fixtures located in the bathrooms. The chlorine sanitizer concentration in a wiping cloth bucket was found at an excessive 200 PPM. Inadequate lighting was observed in the kitchen area due to a burned-out light bulb. An employee was observed wearing a wristwatch and bracelet while actively working with open food items. A soiled mop was stored directly on the floor rather than in a manner that promotes air drying. Buildup and debris were present on the floors and walls under equipment, behind shelving and in the bathrooms. No written notification regarding the presence of major food allergens was provided for unpackaged food. Bowls used as scoops were stored inside food containers with all sides of the bowls contacting the food product. Food service pans were stacked wet in the warewashing area instead of being allowed to air dry. Soil, dust and grease buildup were found on the walk-in cooler fan shrouds and the hood vent filters. Single-use containers, such as old yogurt containers, were being reused to store other food items. The establishment did not have temperature logs available for any of its time/temperature control for safety (TCS) food items. No calibration logs were maintained for the facility’s temperature-measuring devices.
