There was a visible buildup of soil residues on equipment food-contact surfaces, along with flying insects landing throughout the kitchen and a lack of water to sanitize the space. Numerous flying insects were swarming around and inside an uncovered, filled trash can in the kitchen area. The person in charge failed to demonstrate proper food safety knowledge. There were inadequate plumbing air gaps at multiple kitchen systems, including the prep sink and the three-compartment sink. No approved method of hand drying was available at the kitchen handwashing sink. A refrigeration thermometer was positioned incorrectly next to the compressor fans, sitting upside down and reversed. The entire facility suffered from a complete lack of running water, which left faucets dry and restroom toilets filled with unflushed human waste. Containers of raspberry topping in the kitchen coolers had visible organic growth inside them and lacked required date markings. An accumulation of buildup and debris was found across the kitchen floors. Significant debris and soil residues had built up on nonfood contact surfaces, including the doors, handles and exteriors of the reach-in coolers and cooking equipment. The space’s walls were in disrepair with visible gaps and holes.

Family Market & Grocery, 110 Wyoming SE

The walk-in refrigerator and walk-in freezer had no temperature-measuring devices available to monitor ambient air accuracy. Multiple facility storage rooms to the left, back and right were unorganized, dirty and heavily cluttered with nonfunctional equipment, leftover food items and unnecessary litter. The plumbing behind the back-left ice machine was in disrepair and leaking, which caused standing water to accumulate on the floor. There were no required temperature logs available for time and temperature control for safety frozen food items being stored at the establishment. The facility lacked any documentation, records or standard operating policy to effectively inspect and control pests. Both live and dead pests, including mice, cockroaches, spiders, ants and a grasshopper, were found scattered throughout the facility with no active control methods in place. Dead mice and multiple cockroaches were left to accumulate inside overfull glue board traps placed throughout the building. There was an accumulation of dirt, debris, rodent droppings and dead insects on display shelving, under units and inside the walk-in freezer, where the roof paint was also chipping. No current food service permit was posted in a conspicuous place for public examination. The facility failed to provide a designated storage area for employee accommodations, resulting in personal food being stored directly above facility beverages inside the walk-in refrigerator.

Loyal Order of Moose 1517, 2121 Edith NE (May 20)

An inadequate plumbing air gap was found at two floor drains in the bar area. The food establishment did not have a certified food protection manager on staff. The doors leading to the outside were not tight-fitting and allowed light to be seen through the edges. Half-and-half stored inside the bar area cooler measured at an unsafe temperature of 48 degrees. There was an expired or completely missing testing kit to measure the concentration of the chemical sanitizing solution. The bar area handwashing station was improperly used for discarding drinks. No ambient air thermometers were available inside the walk-in cooler and undercounter cooler to monitor temperatures in the warmest parts of the units.
