Chlorine sanitizer in the main kitchen line bucket measured at 0 PPM. Chlorine sanitizer in the back prep area bucket measured over 200 PPM. Spray bottles containing alleged degreaser and bleach were not properly labeled. Fried chicken wings were not hot held at 135 degrees or above, measuring between 109 degrees and 119 degrees. Hot-holding equipment was insufficient, as chicken wings were left in a fryer basket unable to maintain 135 degrees. The facility did not have a certified food protection manager on staff. Working containers of sugar, flour, cornmeal and spices in the back prep area were not labeled. Cold-holding items measured above 41 degrees, including raw meat, raw shrimp, sliced sausage, sliced carrots, cut lettuce, raw chicken and raw hamburger patties. Raw meats and sauces in the walk-in refrigerator were stored less than 6 inches off the floor. Food temperature-measuring devices were missing or not readily accessible. The facility lacked a designated area for employees to eat, drink or use tobacco products. A soiled, unused mop was stored directly inside the mop sink. Temperature logs were not available for stored and received food items. Calibration logs were not available for temperature-measuring devices.

CR Golf, 9717 Copper NE
A stored slicer, spatula, dicer and ice machine had debris buildup. Chopped green chile was improperly thawing in standing water at room temperature in a prep sink. Dead cockroaches were found on the floor in the storage room next to the employee restroom. Prepared vegetables and cooked meats in the walk-in refrigerator lacked date markings. The person in charge was unable to demonstrate required food safety knowledge. Employees lacked valid food handler cards. The facility did not have a certified food protection manager on staff. Bulk working containers of salt and sugar in dry storage were not labeled. Unused and nonfunctional items cluttered the catering storage room and banquet hallway. The facility lacked a designated area for employees to eat, drink or use tobacco. The facility provided no lockers or personal storage facilities for employee belongings. An employee prepared food entrees without wearing a proper hair restraint. An employee ate inside the kitchen, and employee food was stored above facility food and condiments. Employees kept unapproved, open drinks stored throughout the facility. An employee ate inside the kitchen and returned to work without washing hands. Buildup and debris were observed on kitchen walls, stainless steel walls, and underneath shelving and prep tables. Synthetic wood kitchen drawers and cabinets storing utensils had debris buildup. Temperature logs were missing for TCS food items including meats, chiles, vegetables and dairy. Calibration logs were missing for temperature-measuring devices.

Curry Leaf, 6910 Montgomery NE
Raw chicken was prepped while an employee sat on a five-gallon bucket with a bus tub on the floor. Chlorine sanitizer in the automatic dish machine measured at 10 PPM. Chlorine sanitizer in red buckets measured over 200 PPM, and buckets were stored directly on the floor. Stored knives had debris buildup. Eyeglass cleaner was stored inside a soup bowl in the clean dish area. Glass cleaner was stored next to stainless steel pans and above dishware. Foam sealant was stored on a prep table next to raw chicken. Raw chicken and raw goat were improperly thawing at room temperature in a sink. Dead cockroaches accumulated in traps under the dish machine and in the dining area. Substantial rodent droppings were found on shelving above chemicals. Prepared vegetables, soups, cooked meats, sauces and cooked potatoes in reach-in coolers lacked date markings. Cutting boards on prep units had deep scratches and scoring. The person in charge failed to demonstrate required food safety knowledge. The facility lacked a certified food protection manager on staff. Food employees lacked valid food handler cards. Hood ventilation filters were damaged with substantial gaps and debris buildup. Working containers of food were unlabeled, and old ticket chits were stored inside spices. Cold-holding items measured above 41 degrees, including prepared meat, raw shrimp, hummus, chopped green chile, curries and cooked meat. A bag of onions and raw chicken were stored less than 6 inches off the floor. Food temperature-measuring devices were missing or not readily accessible. The facility provided no lockers or storage facilities for employee personal belongings. The main kitchen handwashing sink lacked hand soap. The main kitchen handwashing sink lacked an approved hand drying method. The main kitchen handwashing sink was blocked with duct tape and non-operational due to leaks and no cold water. Thermometers were missing inside cold-holding units. An employee wore a watch while preparing raw chicken and entrees. Multiple food employees prepped food without hair restraints. An open refillable water bottle was stored on a cutting board. The facility was operating with an expired permit. No employees washed their hands when required during the inspection. Buildup and debris were found under stove tops, fryers, dish machine, shelving and cold-holding units. Temperature logs were missing for TCS food items.

The facility had a severe cockroach infestation. Cold-holding display units were not maintaining food temperatures at 41 degrees. Chlorine sanitizer solution was not properly tested and held at 100 PPM.

Eat My Thai Cuisine, 5303 Gibson SE
Active cockroaches were observed throughout the kitchen and facility. Cockroach activity was located at wok stations, prep tables, dry storage, behind the dish pit, the boiler room and front storage. A reach-in cooler measured at 46 degrees. Multiple TCS food items lacked production or expiration dates.

Golden Chopsticks, 12251 Academy NE
A can opener blade in the back prep area had dried, caked-on food debris. Spray bottles containing degreaser and rubbing alcohol were not labeled. Soiled wiping cloths were stored on prep tables and cutting boards. All TCS food items in walk-in and prep top coolers lacked date markings. Food bins of sugar, flour and corn starch near the walk-in cooler were not labeled. Raw meat in a bag was stored directly on top of lo mein noodles in the walk-in cooler. Raw chicken was stored directly above cooked chicken in the walk-in cooler. All food items in the walk-in cooler were stored uncovered. Food temperature-measuring devices were missing or not readily accessible. Testing kits for measuring chemical sanitizer concentration were missing. The main cook line handwashing sink had heavy debris buildup. The bathroom handwashing sink lacked a handwashing sign. Chlorine sanitizer in a wiping cloth bucket measured over the maximum allowed limit at 200 PPM. An employee wore unapproved bracelets and watches while handling open food items. No employees washed their hands during the inspection, including after changing tasks, smashing trash or sweeping. No written notification of major food allergens was provided for unpackaged food. Cups were stored inside food bins with handles directly touching food near the walk-in cooler. To-go boxes were stored directly on the floor in the back of the dining room. Single-use raw chicken boxes were reused to store cooked chicken in the walk-in cooler. Temperature logs were missing for cooled and stored TCS food items. Calibration logs were missing for temperature-measuring devices.

Royal Hyderabad, 5760 McMahon NW
Soiled spatulas, plates, prep tables, make tables and cutting boards were dirty or stored with clean utensils. Lollipop chicken was held past expiration, and fish, sauces and dressings lacked date markings. The scrapper sprayer sink faucet was leaking at the hot and cold water valves. Cooked sauces were left out cooling improperly at room temperature. The facility did not have a certified food protection manager on staff. Intake and exhaust air ducts throughout food service areas had debris buildup. Cold-holding items measured above 41 degrees, including yellow tofu soup, yellow cauliflower, orange sauce and green peppers. Raw chicken left out on a prep table measured 53 degrees. Uncovered chicken drumsticks, sauces and vegetables were stored in contact with undrained ice. Hand-drying towels were missing from all three restrooms. An uncovered employee drink containing a black liquid was stored on a kitchen prep table. Water from a ceiling vent dripped directly onto a pan of cooked chicken in the walk-in freezer. In-use utensils were stored on dirty table surfaces during pauses in use. Stainless steel tables, shelving, storage racks, make tables, stoves, sinks and dishwashers had soil buildup. Floors, walls, drains, cooler areas and freezer areas had debris buildup. Ceiling tiles were missing in the kitchen in front of the freezer. Temperature logs for TCS foods were missing.
